Dorsalgia: Definition, Classification, and Onset

Dorsalgia is defined as pain experienced by the patient in the back, typically along the thoracic spine, and it may radiate to the thorax. The pain associated with dorsalgia is often intense and referred to other regions of the body, which can complicate the process of identifying the underlying cause.This condition can affect individuals of all ages, including younger populations. Certain factors may increase the likelihood of developing dorsalgia, such as tall stature, intense physical activity, or psychological stress (e.g., anxiety or emotional distress).

 

Etiology

In the majority of cases, dorsalgia is not attributed to serious underlying conditions. However, it may occasionally be a symptom of a more severe pathology, such as a cardiac condition, which manifests as referred pain to the back. In such instances, additional symptoms are typically present.The most common causes of dorsalgia are dysfunction of the thoracic vertebral and costovertebral joints. Additional contributing factors may include:

  • Muscle overuse and fatigue
  • Myofascial tension
  • Ιnjuries
  • Poor posture
  • Mechanical strain
  • Other musculoskeletal factors

Management

Upon clinical evaluation including a detailed medical history, physical examination, and, if necessary, imaging studies such as X-ray or MRI,the physician determines the appropriate therapeutic approach based on the underlying etiology.The treatment of dorsalgia may involve:

  • Physiotherapy program focused on strengthening and flexibility
  • Strengthening exercises
  • Aerobic training
  • Surgical intervention, in selected cases with specific indications

With appropriate management, dorsalgia typically resolves within a short period, usually 1 to 2 weeks. In rare cases, the condition may persist for a longer duration. Since dorsalgia can be influenced by multiple factors, including psychological stressors, it may recur under specific circumstances.Recognizing this characteristic of dorsalgia can help patients avoid unnecessary anxiety regarding the natural course and prognosis of the condition.
