Elbow Bursitis: Causes and Management

Olecranon bursitis, commonly referred to as elbow bursitis, is a condition affecting the posterior aspect of the elbow. It is characterized by inflammation of the olecranon bursa, a small fluid-filled sac located at the back of the elbow joint. The condition may occur in either arm but is more frequently observed in individuals over the age of 40. Depending on the severity and onset of symptoms, it can be classified as acute or chronic.

Causes of Olecranon Bursitis

The most common factors contributing to inflammation of the olecranon bursa include:

  • Prolonged pressure on the elbow (e.g., leaning on the elbow for extended periods)
  • Inflammatory conditions, such as arthritis
  • Direct trauma to the elbow, including contusions or blunt injuries
  • Underlying medical conditions, including autoimmune disorders such as rheumatoid arthritis
  • Bacterial infection or contamination of the bursa

Symptoms of Olecranon Bursitis

The most common clinical manifestations of olecranon bursitis include:

  • Swelling in the affected region.
  • Visible swelling or lump at the posterior aspect of the elbow
  • Pain in the affected joint
  • Accumulation of purulent fluid (in cases of infection)
  • Localized erythema (redness)
  • Increased temperature over the affected area
  • Reduced range of motion or difficulty in moving the elbow
  • Generalized fatigue or weakness
  • Fever
  • Excessive Sweating
  • Loss of appetite

Diagnosis of Olecranon Bursitis

Accurate diagnosis is essential for effective management, as olecranon bursitis may also be secondary to an underlying condition. The diagnostic process typically includes Comprehensive clinical examination,Ultrasound imaging of the elbow,Magnetic resonance imaging (MRI),Aspiration of bursal fluid for laboratory analysis,Plain radiography (X-rays).Through this thorough diagnostic approach, the physician can determine the exact cause and establish the most appropriate treatment plan.

Treatment Options for Olecranon Bursitis

Olecranon bursitis can be managed through a combination of conservative and, in rare cases, surgical interventions, depending on the underlying cause and severity of the condition. The most common treatment methods include:

  • Immobilization of the elbow, typically using a simple bandage or, less frequently, a plaster cast.
  • Topical application of anti-inflammatory ointments to reduce pain and swelling.
  • Pharmacological treatment with antibiotics in cases of infection.
  • Physiotherapy program focused on stretching and strengthening
  • Surgical intervention, involving bursectomy (removal of the inflamed bursa) and surgical debridement, indicated only in cases where conservative treatment fails or when the condition recurs at the same site.

In the majority of cases, symptoms resolve within approximately two weeks, depending on the type and severity of the bursitis. Early medical evaluation and intervention are crucial for optimal recovery and to prevent complications.
