A meniscal tear is one of the most common knee injuries. While it can occur in anyone, the condition is particularly prevalent among athletes, especially those participating in team sports such as soccer and basketball, which involve sudden twisting and bending of the knee.The menisci — the medial and lateral meniscus — are firm anatomical structures composed of water and collagen fibers. They are responsible for shock absorption and play a crucial role in maintaining knee stability. When a meniscus is torn, its structural integrity is compromised, rendering it unable to perform its normal biomechanical functions.
Etiology of Meniscal Tears
Meniscal tears are common knee injuries with several potential causes, including:
- Sports-related trauma
- Minor injuries, such as deep squatting, especially in individuals over 55 years old
- Previous knee injury
- Obesity
Clinical Presentation
A meniscal tear is typically associated with acute pain and sometimes a tearing or popping sound at the time of injury. Additional symptoms may include:
- Popping or tearing sensation in the knee
- Pain during walking, running, or climbing stairs
- Night pain
- Burning sensation in the knee
- Swelling in the affected region.
- Stiffness
- Knee instability
- Loss of range of motion
Management of Meniscal Tears
The appropriate management of a meniscal tear is determined after confirming the diagnosis. Following a clinical examination and patient history, the tear is typically confirmed through radiographs and magnetic resonance imaging (MRI).Once the diagnosis is established, the physician selects the most suitable treatment strategy based on the severity and location of the tear. In general, management options include:
- Rest to minimize stress on the affected joint.
- Cryotherapy
- Compression and elevation
- Pharmacological therapy, typically nonsteroidal anti-inflammatory drugs (NSAIDs)
- Individualized physiotherapy program
- Surgical management is most commonly performed via arthroscopy, which is considered the modern, rapid, and safe method for restoring knee function. The procedure involves two small incisions, through which the orthopedic surgeon repairs or trims the damaged meniscus to restore knee mechanics. After the procedure, the incisions are sutured and dressed, and the patient may often be discharged on the same day.
Postoperatively, the patient will require a structured physiotherapy program to fully restore knee functionality and ensure optimal recovery.
